How to file a claim for benefits?
It’s easy; send us a copy of the Notice of Investigation and Certified Suspension Letter. We will let you know if a transcript of investigation is needed. If you choose to sign a waiver, mail or fax it to (229) 424-9613. You never have to pick up the phone. Because each claim is unique, the claims process may vary. The amount of time required to handle each claim depends on the severity, reasons and facts concerning suspension, dismissal, accident or sickness.
